Please use the form below to submit changes about your supervisory union contact information.
We will review your submission and make the changes required after confirmation it is legitimate.
If you need to add another person to a role, such as another data manager, please reach out directly to the SLDS support group directly and we'll assist you.
If a school is closing, please change it's name to "CLOSED" and submit the form, no need to delete grades, etc.
